Mission City Community Network (MCCN), a nonprofit 501(c)(3) corporation, has a 25-year history of providing culturally competent and age-appropriate medical, dental, and mental health services to low-income and medically underserved residents of Southern California.

MCCN operates under a Patient Centered Medical Home (PCMH) model of service delivery. The American Academy of Family Physicians define a PCMH as “an approach to providing comprehensive primary care for children, youth and adults” in “a healthcare setting that facilitates partnerships between individual patients, and their personal physician
